Medical Documentation & Reports

Thorough, accurate medical records that support your personal injury claim or case.

What We Provide

Records Built for Claims and Litigation

Every visit at Twin Cities Chiropractic Spine and Injury Center is documented with exam findings, treatment provided, and progress notes. For personal injury patients, we can prepare narrative reports summarizing the course of care, current status, and Dr. Larsen's clinical findings — formatted for use by insurers, adjusters, or in litigation.

Requests for records or reports can be made by the patient or, with proper authorization, directly by an attorney's office.

Requesting Records

How Records Requests Work

Most documentation requests move through our office within a standard turnaround window once the required authorization is on file. Rush requests can sometimes be accommodated for approaching deadlines — contact our office directly to discuss your timeline.

All records are released only with proper patient authorization or a valid subpoena, consistent with HIPAA requirements.
